Ninety-five percent of information that commissioners need is kept in GP surgeries on a variety of sometimes outdated and unconnected computer servers. This information is largely inaccessible.

A good example was the discovery of a patient who visited A&E 279 times in one year, costing their Primary Care Trust more than £23,000 in hospital attendance charges. This cost didn’t even include treatment.
