Key Features of Population Health Management

  • Seamless access to a combination of HES / SUS and GP data1.
  • Open Architecture. It's your data. We don't take it away from you to work with it, and we place no restriction on what you can do with it.
  • Online reporting: state-of-the-art architecture means there's no batch processing or no "staging" of data: it's all online all the time.
  • Built-in user management which takes into account the complexity of UK commissioning organizations and enables sophisticated access management. Who-sees-what can be controlled based on surgeries, groups, user roles, or individual users.
  • Compliant with IG requirements, specifically the NHS IG Toolkit Primary Care Trust Version 8 (2010-2011).
  • Drill down from any cohort on any chart to view patients and episodes, then print or export the resultant data set.
  • Comprehensive demographic, Long Term Condition, and Care Pathway reporting.
  • Full access to and management of practice and Polysystem financial data at the surgery and commissioning organization. Comprehensive financial reports with drill down through individual patient episodes to all patient data.
  • Rich set of Vital Signs reports which enable monitoring of performance metrics at practice, Polysystem or PCT level. All users share the same view of the data, and practice staff can drill-through to lists of patients.
  • Identification of incomplete Secondary Care Spells, A&E episodes during surgery hours etc.
  • State-of-the-art patient Risk Stratification.

Customization

Our standard software has been designed to lead the field: it should be all you need. If customers have specific requirements to extend our products, our UK-based Professional Services people are ready to help.

Example projects include:

  • Provision of unified reports/ views across SUS and GP data.
  • Custom reports for example:
    • Tracking specific immunization or similar programmes and initiatives
    • Monitoring and cleansing data across SUS and GP systems
  • Integration with third party systems for example finanial systems
  • Support for various patient Risk Scoring algorithms

Support

Health Analytics provides 24-hour support for our customers from our offices in Cambridge UK.

Support includes the following, where appropriate:

  • Regular import, validation and scoring of GP episode data. This is a largely automatic process.
  • Monitoring system performance including machine loads, response times, and Model output.
  • Ensuring that system users can review the latest model reports online.
  • Providing email/ phone/ on-site support for the system as required.
  • Product updates.
  • Tracking changes to source data, for example Read Codes and ICD10 codes etc.
  • Managing user access and general user support issues (PCT and GPs).
  • Monitoring and updates to cope with "non standard" GP codings as necessary.
  • User training as required.

We need controlled remote access to our systems in order to be able to support them in a timely manner. Where systems are running in PCT data centres we typically negotiate limited VPN access to enable us to do this.

Licensing

Our products are licensed on a per-commissioning organization basis with pricing being dependent upon the number of patients and surgeries supported.

Footnotes

1. Requires online access to the PCT's existing HES / SUS data. Assumes use of our GP Data Acquisition tools or the equivalent from another supplier.
